Vaccination Information & Requirements

All children entering kindergarten must be 5 years old before September 1. Parents must bring the child's birth certificate and proof of immunization.

All children must have the following immunizations:

  •  4 doses DPT (one dose after the 4th birthday)
  •  4 doses of Polio
  •  2 doses of MMR
  •  3 doses of Hepatitis "B"
  •  2 doses of Varicella (Chickenpox)
